Responsibilities

JOB PURPOSE

The Support Worker will provide a comprehensive care support service to disabled young people and adults. The role will involve supporting disabled people with individual needs in a social group setting. The aim of the project is to provide disabled people with an opportunity to develop their skills, confidence and independence through meaningful and fun activities.
We require a support worker who can drive our fully accessible miinibus, bringing members into the centre and home again.
There is no personal care involved in the support we offer as indivduals requiring this will have thier own support staff.

KEY RESPONSIBILITIES

  1. To plan and organise activities.
  2. To liaise with family members/carers and or external agencies as required.
  3. To cooperate with staff and other team leaders to assist in the person centered planning process.
  4. To ensure incident reports are completed appropriately and in a timely manner
  5. To ensure the accident book is completed when necessary
  6. To ensure risk assessments are completed appropriately.
  7. To ensure that yourself, other staff and volunteers adhere to organisational policies and procedures as well as relevant regulatory and legislative requirements.
  8. To support and encourage members to participate in services that will promote integration and develop confidence and self-esteem.
  9. To maintain an understanding of relevant issues and developments within the sector
  10. To contribute to the monitoring and evaluation of the services/activities
  11. Any other duties appropriate and reasonable to the post and in line with the needs of the project.
