We are looking for a Support Worker to join our clients team and provide high-quality clinical care and support for a young man.

ABOUT YOU

We are looking for someone who is:
Caring and empathetic – with a genuine desire to support others
Reliable and patient – ensuring individuals receive consistent and quality care
Good at communication – both verbal and written
Flexible – able to work shifts, including days (06:45-19:15), nights (18:45-07:15) and some bank holidays

REQUIREMENTS

· Experience in a similar role is desirable but not essential – full training provided
· A relevant qualification in Health & Social Care is desirable but not essential

· A driving licence and own a vehicle to get to work is essential.

  • Willing to drive the Motability vehicle-VW transporter medium wheelbase is essential

· Must pass an enhanced DBS check

Responsibilities

ABOUT THE ROLE;

As a Support Worker, you will help care for this individual with clinical support, all daily tasks, personal care, and social activities in a dignified manner. Your role will be crucial in empowering the individual to reach his full potential and enhance his quality of life.

KEY RESPONSIBILITIES

· Provide holistic person centred personal care and complex clinical support tailored to this young person’s needs, accompanied to hospital appointments.
· Assist with daily living activities including School/College, outings.
·Access the community and engage in social activities/holidays
·Promote choice in a safe and supportive environment, learning how to interpret his communication
·Maintain accurate records and follow care plans
·Work collaboratively with the young person’s family, healthcare professionals, and colleagues
