We are seeking a reliable and compassionate Disability Support Worker to join our team in a full-time position based in Cockburn Central, Western Australia.

About the position

The Disability Support Worker will provide direct support to people with disability and assist clients with daily living, personal care, community participation and other activities in accordance with individual support plans.

The successful applicant will work closely with clients and relevant healthcare and support professionals to provide safe, respectful and consistent support.

Key responsibilities

  • Provide person-centred support to clients with disability.
  • Assist clients with personal care and activities of daily living.
  • Support clients with meal preparation, shopping and household tasks.
  • Assist clients to attend appointments and participate in community activities.
  • Encourage and support clients to maintain and develop their independence.
  • Follow individualised care and support plans.
  • Monitor and report changes in clients' health, wellbeing and behaviour.
  • Maintain accurate client records, progress notes and other required documentation.
  • Communicate appropriately with clients, families, carers and other service providers.
  • Follow workplace health and safety, infection control, privacy and safeguarding requirements.
  • Report incidents, risks and changes in client circumstances in accordance with organisational procedures.
  • Work cooperatively with other members of the healthcare and support team.

Skills and experience

To be considered for this position, applicants should have:

  • Relevant experience in disability support, community care, aged care or a related support role.
  • Relevant qualification such as Certificate III in Individual Support (Disability), Certificate III in Individual Support (Ageing and Disability), or an equivalent qualification, where applicable.
  • Demonstrated understanding of person-centred support.
  • Ability to provide respectful and appropriate personal care.
  • Good communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Ability to follow care plans, workplace procedures and client instructions.
  • Good record-keeping and organisational skills.
  • Current First Aid and CPR certification, where required.
  • NDIS Worker Screening Check and other required clearances.
  • Current driver's licence and reliable transport, where required for the position.
