Our dedicated team of Kaimahi all work towards a shared mission: empowering our tangata whai ora to lead well and fulfilling lives with maximum independence within their homes and community. In our community mobile services, we are committed to providing compassionate care and support to help individuals achieve their recovery and overall well-being goals. You will be responsible for planning your day, travelling between appointments, and responding to changing needs in real time while maintaining accurate and timely documentation. As a Support Worker, your responsibilities will include: Advocate for tangata whai ora to achieve equitable access, wellbeing, and independence Deliver recovery-oriented, strengths-based support in all interactions Provide safe and effective medication support in line with service guidelines Manage and prioritise a caseload, planning and delivering support across the community Work autonomously in a mobile setting, adapting to changing needs and risks Identify and respond to changes in wellbeing, escalating concerns appropriately Maintain accurate documentation and work collaboratively with clinical and external teams Permanent Shift Pattern: Friday, Saturday, Sunday and Monday 07:30am to 04:00pm working 32 hours per week. About You | Mōu We are interested to hear from people who are driven to support every person using our services to achieve equitable access and outcomes. Current Full New Zealand Driver Licence and ability to work across community settings Current First Aid Certificate or willingness to obtain upon commencement NZ Certificate in Health and Wellbeing Level 4 (or equivalent), or willingness to complete within two years Ability to work independently with strong time management and organisational skills Sound judgement and ability to remain calm in dynamic or higher-risk situations Strong communication skills with the ability to build trust and rapport Cultural competence, emotional resilience, and ability to maintain professional boundaries Closing Date | Rā Katinga: 01 June 2026.
