We are seeking enthusiastic Support Workers – Group Based & Individual Support to facilitate programs that help people with disability connect with their community, build social skills, and work towards their NDIS goals.

Part of Lutheran Services the Keystone Centre offers support programs for people living with a disability. We offer a range of services including a community and centre group based supports, in-home support and community access. With a mix of education, goal-oriented self-development programs and life skills lesson we help our participants and their families imagine the possibilities and then achieve them.
Lutheran Services is a not-for-profit organisation, providing exceptional care and support to communities across Queensland for the past 90 years. We provide aged care, retirement living and home care services for older people, with community services including mental health, disability, youth and family, and domestic and family violence support. • Plan and deliver group based supports for centre-based and community groups.
• Contribute to developing and implementing participant centred individual support plans to support in achieving their NDIS goals
• Support participants one-on-one within their home and through community access to build confidence, skills and independence to continue living within their community.
• Be a positive role model, encouraging independence and confidence.
• Work flexibly, providing centre-based community based and in-home supports.
