Covering North Norfolk and Broadland our service provides an integrated Health and Social Care approach to assessment and support in the community. Norfolk County Council and Hertfordshire Partnership university NHS Foundation Trust (HPFT) deliver together a single service for adults with a Learning Disability. We are an integrated Health and Social Care team providing specialist Learning Disability support to adults in the community. You will need to have a positive attitude and enjoy finding creative and innovative solutions. You will have excellent communication skills, record keeping and keyboard skills. You should have a commitment to excellent customer service and multi- agency working.
