Description Job Posting: Supported Community Connections Manager Location: Starpoint Administration Building Schedule: Monday – Thursday 7am-5:30pm Reports To: Director of Day Services Benefits · Paid Personal Time Off (PTO) · Medical, Dental, and Vision insurance benefits at a minimal cost to the employee · Group Term Life and AD&D Insurance · 100% employer-paid Employee Assistance Program (for employees and their immediate family members) · Matching 401(k) retirement plan Position Summary The Supported Community Connections (SCC) Manager oversees the daily operations of Starpoint’s Supported Community Connections program, ensuring individuals with intellectual and developmental disabilities receive the support they need to thrive in community-based settings. This role provides leadership to staff, fosters strong community engagement, and ensures compliance with all applicable policies and standards. The SCC Manager plays a vital role in helping participants achieve their personal goals, build meaningful relationships, and actively participate in their communities. Essential Duties & Responsibilities Program Development & Community Engagement · Identify and develop community-based opportunities that reflect the interests of participants. · Design and implement stimulating activities that foster growth, inclusion, and independence. · Encourage individuals to develop natural supports and meaningful community relationships. Collaboration & Communication · Serve as the primary contact for families, case managers, guardians, and other stakeholders. · Facilitate development and implementation of Person-Centered Individual Service Plans (ISPs). · Ensure accurate documentation of ISP progress and outcomes. Staff Supervision & Training · Supervise and guide Direct Support Professionals (DSPs) to ensure quality services. · Conduct regular staff meetings, trainings, and orientations for new or relief staff. · Oversee scheduling to ensure adequate coverage and program consistency. Program Monitoring & Quality Assurance · Collect and analyze data to measure progress and inform program improvements. · Promote consumer rights, dignity, and choice in all service delivery. · Apply quality improvement strategies to strengthen program effectiveness. Community Participation & Advocacy · Research and expand opportunities for community integration. · Advocate for independence, self-determination, and participant decision-making. Compliance & Reporting · Ensure compliance with Starpoint policies, Medicaid Waiver requirements, OSHA standards, and all other licensing or contractual obligations. · Prepare and submit required reports in a timely manner. · Participate in management and staff meetings. · Other Duties · Perform additional responsibilities as assigned to support program and organizational success. Supervisory Responsibilities · Direct supervision of a team of Direct Support Professionals (DSPs). Requirements Qualifications Education & Experience · At least 1 year of experience supporting individuals with intellectual and/or developmental disabilities, or completion of relevant training/certification. Skills & Abilities · Strong verbal and written communication skills with staff, families, and stakeholders. · Ability to read and interpret procedures, safety rules, and manuals. · Proficiency in basic math; ability to compute percentages and interpret data. · Problem-solving skills and ability to follow multi-step instructions. Licenses & Certifications · Valid driver’s license with clean driving record. · Access to an insured, registered private vehicle. · Current CPR/First Aid certification (or ability to obtain prior to hire). Physical Demands & Work Environment · Ability to lift/move up to 50 pounds and provide physical support for individuals weighing over 200 pounds. · Regular standing, walking, and communication required. · Exposure to outdoor conditions and moderate noise. · Flexibility to work across community and residential settings.
