Description Ensure compliance with all MTC policies and procedures, requirements of APD and other regulatory agencies. Communicate and Coordinate with Personal Support services where applicable. Document services are provided accurately and in a timely manner. Maintain and update all required documentation in assigned case files in a timely manner Develop implementation plans with individual's input and provide needed support to each individual to achieve stated goals. Maintain regular direct/in person contact with assigned individuals Provide support to assist individuals in achieving stated goals and outcomes. Provide back-up Supported Living Coaching as assigned. Ensure Satisfaction and Choice surveys are completed for each individual on an annual basis. Assist the individuals to develop and maintain appropriate personal care skills and safety skills. Assist the individuals with community inclusion, medical appointments, prescriptions, shopping, money management, transportation, and third-party benefits. Complete and maintain current required training status. Maintains a collaborative work environment and works well with others. Treat everyone with civility, kindness, active listening, and professionalism, even in challenging situations. Maintains a collaborative work environment and works well with others. Other duties as assigned. Requirements Bachelor’s degree from an accredited college or university with a major in education, rehabilitative science, business or related degree. Or an associate’s degree (or two years of college) from an accredited college or university and have two years of documented direct experience with developmental disabilities. Or one year of college from an accredited college and three years of documented direct experience with developmental disabilities. Or four years of direct professional experience in working with developmental disabilities. (May substitute for college on a year-by-year basis with a high school or GED diploma.) Must have the following skills: Customer Service, Developmental Disabilities, English language, mathematics, person centered model, personal outcomes, adaptive /behavioral skills, self-directed teamwork, and housekeeping. Must be able to meet deadlines. Intermediate computer skills in MS Office, Excel, Word, PowerPoint, and Outlook. Data entry and notes into industry software. Flexible to work nights and weekends as required. Have exceptional interpersonal skills, and the ability to communicate and manage well at all levels of the organization (including staff at remote locations). Is culturally sensitive, disability sensitive, and socially perceptive. Establish and maintain positive and collaborative relationships, both internally and externally.
