The Supports Broker is responsible for coordinating the daily operation of Medicaid Waiver services, and for promoting personal growth and independence of participants in accordance with state and program regulations, GT Independence Policy and Procedures, and applicable accreditation standards.

MINIMUM REQUIREMENTS

  • The Support Broker may not work for the VF/EA FMS-Support Broker entity and

concurrently as a waiver case manager or for a direct care service organization.

  • High school diploma or equivalent
  • At least 1-year community experience supporting people with disabilities

Responsibilities
  • Support participants’ growth and independence by training, supervising, and monitoring staff.
  • Build and maintain relationships with agency staff, service providers, and families.
  • Assist participants in developing social networks and integrating into the community.
  • Help locate and access non-Medicaid resources to achieve Individual Support Plan (ISP) outcomes.
  • Provide instruction and counseling for problem-solving and decision-making.
  • Advocate for participants and collaborate with relevant organizations.
  • Guide participants in planning and implementing support plans (IEP, ISP, etc.).
  • Educate and support participants in self-directing services under the Employer of Record Model.
  • Assist in budget management and coordinate financial support services for self-directed participants.
  • Develop schedules and staff duties for self-directed participants.
  • Provide ongoing training on participant-directed responsibilities and reporting.
  • Coordinate community resources, including healthcare services.
  • Assist in securing housing and necessary furnishings.
  • Meet billable targets.
