This role works under the supervision of a Qualified Supports Coordinator or Manager and adheres to Person-Centered Planning principles and regulatory requirements. ---------------------------------------- Key Responsibilities * Assist with implementing Individual Plans of Service (IPOS) * Link, coordinate, and monitor services and supports * Help individuals maintain entitlements (Medicaid, SSI, etc.) * Support crisis planning and resolution * Coordinate health and safety strategies, including discharge planning * Promote community inclusion, self-advocacy, and relationship building * Assist with housing, transportation, and employment resources * Complete timely documentation and maintain compliance with agency and funder requirements ---------------------------------------- Qualifications * Education: * Bachelor’s degree in Human Services (eligible for advancement to Supports Coordinator after one year) * OR Bachelor’s degree in non-human services field * OR Associate degree or 60 credit hours plus 2 years of experience with individuals with I/DD * Strong communication, organizational, and computer skills * Valid Michigan driver’s license, insurance, and acceptable driving record ---------------------------------------- Additional Requirements * Extensive travel within Wayne County; reliable transportation required * Ability to work flexible hours and maintain documentation standards * Completion of job-specific training provided by CLS ---------------------------------------- About Community Living Services (CLS) CLS provides person-centered supports for individuals with intellectual and developmental disabilities in Wayne County.
