JOB SUMMARY

The Supervisor Comprehensive Care Management in Social Work oversees daily activities and staff. They assess and coordinate patient care, including case management, social work, utilization review, and care coordination. The goal is to achieve optimal clinical and quality outcomes by managing care and resources effectively to reduce unnecessary utilization.

QUALIFICATIONS

  • EDUCATION - Masters’
  • MAJOR - Social Work
  • EXPERIENCE - 3 Years of Experience
  • CERTIFICATION/LICENSE/REGISTRATION - Lic Clinical Social Worker (LCSW), Lic Masters Social Worker (LMSW), Lic Master Social Wrk AdvPrac (LMSW-AP): Must have one of the following: LCSW, LMSW, LMSW-AP. Job requires a master’s in Social Work.
Responsibilities
  • Provides administrative, technical, and clinical supervision of assigned staff. This includes hiring, firing, performance evaluation, education, staff development, work allocation, and problem resolution.
  • Trains and helps staff in identifying needs of the people and families served to ensure quality of care.
  • Participates in development of care plan of services, coordination, delivery, and quality of services oversight.
  • Identifies and implements standards of practice and care to achieve optimal clinical, financial, and patient satisfaction outcomes.
  • Develops relationships and processes to involve physicians, pharmacists, nurses, social workers, and other health professionals in care planning, delivery, and discharge planning.
  • Participates in developing and implementing standardized policies for efficient operations and optimal patient care. Ensures compliance with all institutional policies and regulatory requirements.
  • Oversees the development of discharge plans and strategies that address the patient needs through the continuum of care post-discharge.
  • Ensures appropriate scope of practice by staff. Ensures subordinates know ethical standards like confidentiality, informed consent, conflicts of interest, dual relationships, boundary issues, documentation, consultation, referral, and end of services.
