We establish goal oriented and individualized support that focuses on improving their self-sufficiency. Targeted Case Management is a dynamic process that assesses, plans, implements, coordinates, monitors, and evaluates to improve outcomes of client centered goals. JOB SPECIFIC FUNCTIONS: Conduct new client assessments and develop care plans Consistently execute care plans in a timely, efficient, and equitable manner Collaborate with Clinical, Programs, Education and After care Department team members Assist clients in gaining access to medical, social, educational and vocations services Facilitate multiple care aspects (case coordination, information sharing, etc.) Serve as client advocate in helping them make informed decisions regarding their clinical status and treatment options Proactively seek out client contacts, direct /indirect and document their progress and to ensure success Record and complete assessments, plans, notes, and discharges Adhere to Kentucky Administrative Regulation (KAR) professional standards, rules, and regulations Requirements QUALIFICATIONS: BA or BS degree in a behavioral science or related healthcare field Minimum one year post graduate experience related healthcare or human service field Effective written and oral communication skills Excellent organizational and time management skills Experience working as a Target Case Manager in the SUD field preferred Working knowledge of case management principles, healthcare management and reimbursement. Medicaid knowledge a plus Problem solving skills and ability to multi-task Collaborative and process driven Current TCM (CORE/SUD) certification is a plus
