Job Purpose: The Hospice Team Coordinator provides administrative and operational support to the interdisciplinary hospice team to ensure efficient coordination of patient care services. This role supports clinical staff by managing scheduling, communication, documentation, and workflow processes to promote continuity, quality, and timely delivery of hospice care in compliance with organizational policies and regulatory requirements. Duties & Responsibilities: Assists with answering incoming phone calls, triaging messages, and ensuring timely communication to the appropriate team member. Provides clerical and administrative support, including uploading and maintaining documents in the electronic medical record (EMR) system and distributing requested physician orders, history and physicals (H&Ps), and related clinical documentation. Prepares and distributes the Interdisciplinary Team (IDT) meeting agenda, supports facilitation of IDT meetings, and prints IDT notes for nursing staff to bring to facilities and maintain in patient binders as required. Assembles patient binders and prepares materials needed for meetings, facility visits, and team conferences. Coordinates ordering, repair, and pickup of durable medical equipment (DME) as needed to support patient care. Notifies pharmacy of all patient admissions by transmitting required documentation, including patient face sheets, and communicates all patient deaths in a timely manner to ensure appropriate medication management and billing closure. Manages supply ordering and distribution to ensure clinical staff have necessary materials to deliver care safely and efficiently. Additional duties as assigned. Duties and responsibilities may change, and new ones may be added, at any time with or without advanced notice. Skills and Qualifications: High school diploma or equivalent required. College graduate preferred. Excellent interpersonal and organizational skills and the ability to communicate effectively. Demonstrate proven decision-making skills. Requires a valid Drivers License. Reliable transportation and valid auto liability insurance. Working Conditions: Works in a routine office environment. Noise level may be moderately high. Must be able to work a flexible schedule and extended hours. Ability to travel locally as needed. May experience exposure to unpleasant weather conditions. Physical & Mental Requirements: Prolonged sitting and some standing required. Requires the occasional need to lift, pull, carry, and push items weighing up to 50 lbs. Requires the frequent need to stoop, kneel, and reach while accessing files. Requires working under some stressful conditions to meet deadlines and employer and employee needs. Requires eye-hand coordination and manual dexterity. Requires problem solving skills.
