Description

NJ Palliative Care, in partnership with RWJ Hospital, is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to take on a blended role that combines direct patient care with clinical team coordination responsibilities.
This position allows you to remain actively involved in patient care while serving as a point of contact for fellow APPs, helping to coordinate schedules, workflows, and communication between the clinical team and leadership.

QUALIFICATIONS:

  • Active NJ NP or PA license and board certification
  • Strong clinical skills and ability to manage complex patients
  • Prior palliative care or hospital medicine experience preferred
  • Excellent communication and organizational abilities
  • Interest in mentoring and helping coordinate a clinical team
  • Compensation note: Candidates considered for the higher end of the pay range are expected to have 10–15+ years of palliative care experience in addition to strong leadership and mentoring abilities

Responsibilities
  • Provide inpatient palliative care assessments and follow-up visits
  • Develop and implement individualized care plans in collaboration with the care team
  • Manage patient symptoms including pain, shortness of breath, and other distressing issues
  • Facilitate goals-of-care conversations with patients and families
  • Serve as the first point of contact for APP team members regarding scheduling, daily workflow, and case coordination
  • Assist in onboarding and mentoring new APPs
  • Support quality improvement initiatives and track key performance metrics
  • Coordinate with program leadership on staffing and operational needs
