We know that working together is the only way we can achieve what is necessary for a healthier future.     About the role     Chandler House Continuing Care Team is a dynamic multidisciplinary team providing case management for people adults with a severe mental illness. We work with consumers, their carers, families and other community agencies to identify and address their needs, coordinate their care and assist in recovery.      We are seeking a Team Leader who would like to join our team in providing a highly responsive and flexible service for our customers. The Team Leader is the essential clinical operations lead; managing day to day operations, leading incident response, monitoring team performance and leading sustainable, continuous improvement.     About you      To be successful in this role you will possess:     Over ten years working in public mental health in a variety of settings Highly developed interpersonal and leadership experience Demonstrated change leadership experience Current registration with AHPRA, or eligible for AASW membership A strong commitment to high quality care The ability to work as part of a diverse team Excellent communication and organisational skills     Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice).
