About the role Chandler House Continuing Care Team is a dynamic multidisciplinary team providing case management for people adults with a severe mental illness. We work with consumers, their carers, families and other community agencies to identify and address their needs, coordinate their care and assist in recovery. We are seeking a Team Leader who would like to join our team in providing a highly responsive and flexible service for our customers. The Team Leader is the essential clinical operations lead; managing day to day operations, leading incident response, monitoring team performance and leading sustainable, continuous improvement. About you To be successful in this role you will possess: Over ten years working in public mental health in a variety of settings Highly developed interpersonal and leadership experience Demonstrated change leadership experience Current registration with AHPRA, or eligible for AASW membership A strong commitment to high quality care The ability to work as part of a diverse team Excellent communication and organisational skills Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the Australian Immunisation handbook (based on ATAGI advice). Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment.
