JOB DESCRIPTION:

Under the direction of the Director of Care Beyond Walls, the Lifestyle Medicine Team Leader is responsible for executing and maintaining high quality standards of personal performance; accountability for efficient department operations in the lifestyle medicine clinic. Ability to work across multidisciplinary collaborations and execution on Emplify’s enterprise Lifestyle Medicine strategy.

Responsible for the performance of a department/business unit as defined by Bellin’s strategies

Provide leadership to obtain operational and strategic goals within the business unit and affiliated Brand and/or Service Delivery Structure. Promote safety and quality improvement activities and develop people within a team focus. Act as a change agent consistent with Bellin Health System vision, mission, and strategies.

QUALIFICATIONS:

Education: Bachelor’s degree required preferred
License: RN preferred or clinical practice background preferred
Experience: 3-5 years of clinician or clinic management preferred
