Responsibilities

The Team Manager acts as the leader of the Patient Care Team, responsible for supervising, evaluating, and coordinating the various members of the interdisciplinary team. This role ensures continuity of care from admission through discharge or transfer to bereavement, while also serving as a patient advocate and coordinating with external healthcare providers.
