We are looking for a Team Manager to help us deliver our vision of supporting people to achieve what matters to them, living independently in a place they call home.  Collaboration and co-production are at the heart of what we do. Working alongside partners, community organisations, carers, and most importantly the people we support to shape meaningful and personalised outcomes.   The role  Joining our WES (Wellbeing and Early Support) as a Team Manager you will be responsible for leading the team to deliver proactive, strengths-based support that promotes independence and prevents escalation of need. The role involves overseeing assessment and care planning processes, ensuring timely interventions, and embedding a preventative approach in line with statutory duties and local policies. As a Team Manager you will provide supervision, guidance, and performance management to staff, while fostering collaborative working with health, voluntary, and community partners.
