A RN Medical Care Manager contributes to AHF by utilizing strongnursing clinical skills and understanding of the Ryan White Programwithin their specific area. With a caring and knowledgeable focus,you will work with our physician and other providers, the CareManagement team, and the Healthcare Center team so that everyRyan White patient receives the support they require and access toavailable resources. This will include providing education and followupwith the clients. Working directly within the Healthcare Center,you will meet with patients and complete Needs Assessments,develop Care Plans, and provide ongoing education to clients so theyunderstand their diagnoses and their medications. Working with yourteam allows for full collaboration to meet all aspects of your patient’shealth and psychosocial needs. In this way, you will support thepatient’s ability to understand their health status better to work toachieve their healthcare goals. The RNMCM will consult withclinicians to develop treatment regimens that ensure optimal clinicaloutcomes, address treatment adherence education needs and provideongoing education for clients to self-manage their HIV by keepingtheir healthcare appointments and taking their prescribedmedications. You will also conduct ongoing retention calls to ensureno gap in retention efforts and to uphold radical cooperation acrossdivisions. Licenses and Certifications: Bachelor’s degree in Nursing (BSN), orLicense Practical Nurse (LPN) with a minimum of 1 year of CaseManagement experience.Knowledge of Ryan White Services is preferable.
