Description

This position is responsible for conducting face-to-face assessments (virtual assessment) of members who may need skilled nurse assessment services. The incumbent completes all assessments using a skilled care acuity tool and provides a recommendation for services based on members health care needs. Members who are assessed reside in the community or are ready to be discharged from a facility. The incumbent may also serve as mentor or preceptor for new staff, assisting in the formal training of assessors, complete quality assurance audits, inter-rater-reliability reviews, and may be involved in process improvement initiatives.

REQUIREMENTS

  • Four-year nursing degree and at least three years of experience in nursing within the last ten years.
  • Experience working with individuals with disabilities, individuals with intellectual and developmental disabilities (IDD).
  • Experience working with Pediatric and Geriatric populations with various disabilities, and individuals with complex health needs preferred.
  • RN License in good standing.

Responsibilities
  • Performs face-to-face and/or virtual assessments of persons with disabilities. Collaborates with health care providers and ancillary service providers to create or validate the appropriate treatment plan.
  • Documents assessment results for the individual member status or negotiates service plan as needed.
  • Completes quality assurance competency reviews and other tasks/projects as assigned such as new assessor training.
  • Provides ongoing education. May communicate with all health care providers to ensure appropriateness of care.
  • Performs miscellaneous duties as assigned.
