QUALIFICATIONS

  • Master’s Degree in Social Work from an accredited university
  • Current registration with the Ontario College of Social Workers and Social Service Workers
  • Recent clinical experience in an acute health care setting preferred
  • Recent experience with Nephrology patients and skill in crisis intervention and grief work preferred
  • Demonstrated knowledge of relevant health care legislation e.g. Powers of Attorney, living wills
  • Demonstrated strong psychosocial assessment and therapeutic intervention skills related to coping with chronic illness
  • Demonstrated ability to identify and Implement strategies with patient/family by establishing a therapeutic relationship in concert with proven treatment modalities (for example Crisis Intervention, Cognitive-Behavioural Therapy, Critical Incident Stress Management, Solution-focused Therapy, etc.)
  • Demonstrated excellent clinical assessment and treatment skills
  • Demonstrated excellent communication skills (both oral and written)
  • Demonstrated skill in crisis assessment and intervention
  • Demonstrated knowledge of community services and resources and experience with community networking
  • Demonstrated strength in interpersonal relations, problem-solving, crisis intervention, group dynamics, community development and other innovative approaches
  • Demonstrated ability to function independently and interdependently with a diversity of health care team members
  • Demonstrated skill in providing education to health care team and community resources
  • Demonstrated ability to measure and evaluate practice change outcomes
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC
  • Demonstrated practice and commitment to LHSC’s Vision, Purpose and Values
  • Demonstrated ability to attend work on a regular basis
