JOB SUMMARY

Positive Assistance is a not-for-profit organization. Our mission is to assist and give hope to people living with chronic illnesses or at risk of getting chronic illnesses. This position is responsible for contributing to Positive Assistance’s Value Based Care measures. This includes but is not limited to participating in related projects/initiatives, compliance, auditing and medication management.

SKILLS

  • High School Diploma/ GED
  • Certified Pharmacy Technician and/or Certified Medical Assistant, preferred
  • 2 years of medication management experience or pharmacy tech experience.
  • 2 years of experience in a healthcare setting
  • Strong analytical skills with attention to detail and ability to translate data into a quality analysis.
  • Excel experience required
  • Superb time management skills
  • Reliability, productivity, creativity, punctuality
  • Excellent customer service skills
  • Speak, read and understand the English language
  • Self-motivated, action-oriented, strong drive with the ability to make decisions, act independently, and meet guidelines
  • Medication insurance billing and medication claim submission experience
  • Certification to perform CPR/BLS
  • Valid FL Driver’s license in good standing
  • Bilingual preferred (English/Creole)

Responsibilities
  • Demonstrates empathy and develops caring rapport with patients. Promotes patients’ independence and self-care
  • Maintains accurate and complete records of medication delivered to clinic and to patients.
  • Communicated with pharmacy staff and providers on medication inaccuracies.
  • Communicates any changes in a timely and effective manner to management pharmacy.
  • Analyze spreadsheets and patient data
  • Provide outreach efforts to those patients who are at risk of falling out of care by scheduling appointments and providing prescription refills as needed
  • Provide support in medication adherence opportunities as well as other pharmacy related needs
  • Serve as liaison between Providers, Case Managers, PrEP Coordinators and Pharmacies to ensure that patients receive their medication within 72 hours.
  • Follow newly diagnosed patients for the first 90 days to make sure that they don’t fall out of care after they initiate. Help them navigate the complicated health care system.
  • Follow up with nPEP patients to make sure that they receive their full treatment and encourage them to continue with their preventative care as a PrEP patient.
  • Monitor all incoming requests for prescription renewals (i.e. from electronic renewal requests, pharmacies, and patients) and response as per protocol within 48 hours of receipt.
  • Identify requests that fall outside prescription renewal guidelines and seek assistance from pharmacists and providers, when appropriate
  • Take proactive actions to ensure patients’ continued accessibility to prescriptions. These actions include but not limited to making an appointment, providing a refill, communicating with provider, and obtain lab orders when necessary.
  • Maintains accurate and updated documentation for all activities in Positive Assistance’s EMR.
  • Communicate effectively and timely with internal and external stakeholders
