Description Greater Nashua Mental Health is looking for a full-time Therapist to join our Deaf & Hard of Hearing Services team! The Therapist provides direct treatment and case management services to mentally ill Deaf, hard-of-hearing, adults, children, and their families. Delivers consultation and education to staff, other agencies, and the community. Duties & Responsibilities: Provides culturally and linguistically appropriate services, such as individual, couples, family, and community-based to individuals who are Deaf and hard of hearing for a percentage of time per week as appropriate for position and as directed by supervisor. Facilitates a consumer-driven recovery focused model of patient care which consists of, but is not limited to, assessment, diagnosis, education, treatment planning, crisis planning and response, interagency collaboration, implementation of best practices, and discharge planning. Implements services in treatment plans as collaboratively developed with the client and the multi-disciplinary team utilizing appropriate, defined treatment interventions in accordance with evidence-based practices. Provides community-based services in accordance with specific prescribed needs in the care plan, which are developed collaboratively with clients, including, but not limited to, case management, functional support, advocacy, treatment coordination with other service providers internally and externally, and providing education to support Deaf and hard of hearing individuals. Ensures that the client's rights are upheld. Completes all treatment plans, Adult Needs and Strength Assessments (ANSA), and Case Management Care Plans (CMCP) by due date listed in EMR. Completes all Quarterly Reviews within a two-week window of the end date of the quarter being reviewed. Completes all other documentation within 3 business days (with first day being date of service/completion). Communicates professionally and effectively with colleagues, collateral contacts, supervisors, etc. regarding client care. Prepares for and attends meetings, including team meetings (1x weekly), therapists' meeting (1x weekly), department meetings (weekly), supervision meetings (1x weekly), etc. Supports the team in outreach, special projects, and the provision of consultation/education to GNMH employees, other agencies, and the community. Manages and responds to email, phone messages, etc. in a timely and professional manner. Performs other duties or special projects as required or as assigned. Requirements Qualifications: Master’s Degree in Counseling, Social Work, or related field Clinical licensed preferred 1 to 2 years related work experience Fluent in American Sign Language (ASL) Knowledge of resources available to Deaf and hard of hearing population Familiarity with community resources Ability to teach effectively Knowledge of and ability to use Microsoft Office products (Word, Outlook, Excel, etc.) Strong collaborating skills and an ability to work effectively both independently and as part of a team Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive Excellent communication and interpersonal skills Excellent organizational and time management skills Ability to manage multiple priorities and be flexible with regard to workload and assignments Ability to exercise sound judgment Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy Why Join Us: We are looking for collaborative, curious for knowledge people with a “can do” mentality to join our team of compassionate employees!
