Families in Transition is currently looking for a Therapist to join our team! The Therapist is responsible for providing assessments, conducting weekly therapy, and facilitation of groups. The therapist will collaborate with community partners in client care, and provide aftercare services. The Therapist is responsible for completing timely documentation, treatment planning for client services. This position allows professional growth opportunities in both the mental health and substance use fields. As a therapist working with these families, a strong collaboration between teams at Families in Transition is crucial. Essential Functions: Attend and participate in monthly staff meeting, weekly team meetings and ongoing clinical supervision. Facilitates intensive outpatient and outpatient groups as assigned throughout the week by utilizing the evidence-based curriculums assigned for that treatment day. Undertaking assessments, including assessment of mental health and substance use needs, risk assessment and formulating a psychological explanation of the client's issues. Knowledge and practice with the DSM5 criteria for substance use and mental health disorders. Evaluating the outcome of therapy. Establishing a collaborative working relationship with the client based on trust and respect. Training and supervision of interns as needed. Continuing personal and professional development while maintaining your licensure through the State of NH. Participate in an integrated health care model, working with participant’s primary health care providers, state agencies, and other collateral contacts within participants’ care. Provide care coordination and case management services based on client’s needs. Practice cultural competency with establishing rapport and relationships with participants from other cultural and ethnic backgrounds. Perform observed drug testing and screening as appropriate. Knowledge of mandating reporting and ability to call appropriate agencies with safety concerns. Knowledge of and abide by the requirements of 42 CFR Part 2 and HIPAA requirements.
