Responsibilities

MAIN PURPOSE:

To support the Therapy staff to provide high-quality holistic person-centred care and support to service users to reach their full rehabilitation potential.

MAIN TASKS AND RESPONSIBILITIES:

  • Together with the multi-disciplinary team responsible to ensure optimum cognitive, social, emotional, physical and functional needs are addressed.
  • To support service users with cancer or other life-limiting illnesses, who have specialist palliative or neurological needs
  • Responsible to ensure that the service users’ individual rights and choices are actively promoted and supported, and their beliefs and preferences appropriately acknowledged
  • Responsible to report status and variances in treatment and condition to the appropriate Therapist at the earliest opportunity and progress treatment as advised by the Therapist
  • Where appropriate, responsible to review, order and fit equipment and work with service users and their carers/family to demonstrate its safe use, either in the centre or in service user’s home
  • Responsible to liaise with service users and family members to help complete forms, gather information and co-ordinate interventions
  • To support the Therapy Team by completing administrative tasks such as gathering referrals, recording monthly activity data
  • Responsible to collect, collate, evaluate and report information, maintaining accurate service user records in all formats, including written and electronic information systems
  • Facilitating the safe and smooth running of the Service, the post holder is expected to maintain a safe working environment, maintain service user confidentiality, and occasionally taking on reasonable additional tasks
