We are seeking a Therapy Program Manager to lead and oversee our therapy services, including Physical Therapy, Occupational Therapy, and Speech-Language Pathology. In this role, you’ll manage the daily operations of the therapy department, ensuring high-quality resident care, regulatory compliance, and staff development. You’ll collaborate with interdisciplinary teams, support residents’ individualized care plans, and provide hands-on therapy services as needed to maintain continuity of care and departmental productivity. Your leadership, clinical expertise, and commitment to excellence will shape outcomes for both residents and the therapy team.

SPECIALIZED KNOWLEDGE, SKILLS, OR ABILITIES:

  • Knowledge of PDPM, MDS, and Medicare/Medicaid reimbursement guidelines
  • Strong leadership, organizational, and communication skills
  • Ability to lead interdisciplinary collaboration and adapt in a fast-paced environment
  • Ability and willingness to provide hands-on clinical care when needed

Responsibilities
  • Track department budgets, expenses, and billing practices.
  • Monitor therapy utilization and ensure accurate documentation for reimbursement (e.g., PDPM compliance).
  • Adhere to infection control policies, professional therapeutic services, and Community First Solution’s standards, policies, procedures, and guidelines.
  • Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.

  • All other duties as assigned and/or appropriate to the position.

  • Qualifications:
    Education: Licensed Physical Therapist (PT), Occupational Therapist (OT), Speech-Language Pathologist (SLP), or Therapy Assistant (PTA/COTA) with management experience.
    Licenses or other required certifications: Ohio License as a Speech-Language Pathologist. CPR certification.
    Experience: Minimum of 3 years of clinical experience in a long-term care or skilled nursing facility
    Prior supervisory or management experience preferred
