Description

An exciting opportunity has arisen for a Band 3 Therapy Support Worker to join our Care of the Elderly Team, to support with the prevention of deconditioning. The post holder will work alongside the rest of the therapy team to deliver a wide range of delegated therapeutic interventions including ; mobility, exercise, personal and domestic care activity and equipment provision. You will have a key role in the prevention of deconditioning of patients across the elderly care wards. You will also work with other health care professionals to raise awareness and educate staff, patients and relatives about the importance of promoting independence and preventing deconditioning during hospital admission.
Applicants should have excellent communication skills, be positive, compassionate, flexible and work well as part of a team.

Core working hours are Monday – Friday, but you will be expected to work any shifts in operation, which may include weekends and public holidays.

  • To use the principles of Occupational Therapy and Physiotherapy in an integrated and client-centered way, to promote independence and facilitate safe discharge. This will be achieved by implementing delegated treatment plans and facilitating rehabilitation within agreed limitations through exercise, physical modalities and the use of occupation.
  • To have a key role in the prevention of deconditioning of patients across our In-Patient wards. You will also work with other health professionals and healthcare assistants to raise awareness and educate staff, patients and relative about the importance of promoting independence and prevention of deconditioning during hospital admissions.
  • To work directly with patients to provide interventions such as exercise programmes, practice self-care and other activities of daily living. You will also provide education and support to patients, relatives and other health care staff on the importance of promoting independence and preventing deconditioning during their hospital stay and beyond.
  • To assess and manage delegated patients with mobility and occupational difficulties, reporting back to the registered practitioner as necessary.
  • To support the teams with administration, clerical and housekeeping duties.
  • To work as an integral member of the therapy team using the principles of Occupational Therapy and Physiotherapy in an integrated and client-centered way. The post holder will implement assessments and treatment plans to promote independence through the use of occupation, exercise and physical modalities, taking into consideration a person’s affect and cognition.
