Description
Responsibilities

An exciting opportunity exists for a trainee to work with The Hospital in the Home. This vocational training Extended Skills post is approved by local Regional Training Providers for General Practice training and for Geriatrics. Hospital in the Home provides care in the community for patients who would otherwise be admitted to an acute hospital bed. The service is designed to give patients the choice of care at home rather than as an inpatient, as well as to reduce the length of hospital stay and avoid unnecessary admissions.

Hospital in the Home provides an acute treatment service for patients with conditions such as pneumonia, cellulitis and anticoagulation needs, a supported discharge service, a multidisciplinary out-patient facility and management of chronic and complex medical conditions. A patient-centred, evidence-based clinical approach is fostered within the service. This term is offered as a GP Extended Skills, Geriatrics or General Medicine position for 6 months from February to August or August to February. Duties include Clinical assessment and management of patients referred to the Hospital in the Home Service. This will include Home Visits and visits to Residential Aged Care Facilities. Liaison with local General Practitioners is a major part of daily activities and the patient handover is a vital part of the continuum of care. Expectations of the trainee include conducting a quality assurance improvement project during the term, participation in current research and multidisciplinary teaching sessions.
