Description The Transition Coach supports people with disabilities transition into competitive integrated employment from the training program to the employer site. The Transition Coach collaborates with Career Navigators to effectively deliver Aspire’s job training program, including classroom instruction, goal development, transition support services and job coaching during an internship. The Transition Coach partners with employers to build embracing and inclusive environments. Position Responsibilities: The following represents the primary position responsibilities. At the same time, other duties related to the organization may be assigned. Partner with Career Navigator to facilitate the training go both simulated work and soft skills development related to career readiness. Partner with Career Navigator to evaluate/assess students for interests, dreams, goals, skill strengths and areas of needed skill development. Identify and assist the students in developing strategies to overcome barriers to employment. Provide one-on-one support during internships and create a fade out plan to lead to independence. Provide support to facilitate successful transition into competitive integrated employment. Provide ongoing job support to ensure the success of placement while developing natural supports at each community-based work location. Create adaptive support as needed, including by not limited to, checklists, social stories, visual prompts and aids. Serve as a liaison between employers and the people at the worksite. Service as a point of contact for students and family members as needed. Reports to this position: None Requirements High school diploma or GED required; Bachelor’s degree in human services, Social Services or Education, preferred. Minimum one (1) year of related work experience Intermediate proficiency with Microsoft Office Valid driver’s license with a good driving record Experience working with adults with disabilities preferred Knowledge of job coaching methods preferred Competencies/Behaviors Communication – effectively communicate (oral and written) across all levels of the organization Maintain Confidentiality Interpersonal – engage with all people, team members, families and outside organizations in an engaging, courteous and tactful manner Organization – establish and manage priorities, mulit-task, concentrate in an environment with frequent interruptions and/ distractions. Facilitation –train in classrooms and simulated environments. Reporting – record information accurately and effectively, attention to detail and accuracy
