Description RESPONSIBLE TO: Director of Health Care Services General Manager PURPOSE: To assist guests and families in the transition from admission to discharge; providing help and support in obtaining the services needed to increase the physical, social and emotional well being of our guests. QUALIFICATIONS: Education: High School diploma is required; associate's degree or higher in the area of human services preferred.. Experience: Knowledge of Medicare and other third party insurance is required. Effective written and verbal communication skills required. Personal Characteristics: Effective written and verbal communication skills. Interpersonal skills required communicating with guests and acting as a facility representative. Person should have a kind demeanor and must be neat and well groomed. Must have the ability to work independently when necessary DUTIES: The duties of the Transition Coord position may include, but are not limited to the following areas: To inform residents and families of services, charges, and facility policies and procedures. Reviews potential referrals for admission. Accepts referrals unless referral is to be reviewed by clinical staff. Ensures proper pre-admission screening is completed and uploaded into the EMR before the guest arrives to the facility. Pre-admission Screening for admission to the rehabilitation/long term care programs Initial PASSAR Chest X-ray History and Physical Medication Lists Complete insurance pre-authorization if necessary for admission To complete admission paperwork as needed for new residents and explain the admission process. Offers Psych services to all guests upon admission, obtains signed consent via document manager and keeps an accurate list of all guest’s participating in Psych services. To communicate guest and family’s social needs to IDT ensuring a successful stay in the facility and reintegration back into their community. To coordinate and inform nursing staff of scheduled admissions and discharges. To organize guest’s discharge process and ensure services and equipment needed are set up/ordered prior to discharge. Maintains an up-to-date knowledge of practice of admission procedure, policies, insurance, Medicare & Medicaid programs Keeps Director of Health Care Services informed regarding resident, family, or staff concerns. Obtains services of dentist, podiatrist, and eye doctor as needed, including eyeglass repair, necessary appointments and transportation. Oversees the completion of concern/complaint reports Oversees room changes including documentation, notification of the guest, moving of all personal items, meds, etc. and notifies nurse receiving resident of health status and preferences. Explains guest bill of rights upon admission Provides guest education on public posting book upon admission Encouraging completion of patient satisfaction surveys WORKING CONDITIONS: Works in office areas as well as throughout the facility and its premises Moves throughout building intermittently during working hours Is subject to frequent interruptions Is subject to call back during emergency Is involved with guests, family members, personnel, visitors, government agencies/personnel, etc. under all conditions/circumstances Is subject to hostile and emotionally upset guests, family members, personnel and visitors Is subject to exposure from infectious waste, disease, conditions, etc, including TB and the AIDS and Hepatitis B Viruses Communicates with the medical staff, nursing personnel and other departments or supervisors Maintains a liaison with the guests, their families, support personnel, etc., to assure that the guests needs are continually met PHYSICAL AND SENSORY REQUIREMENTS (with or without the aid of mechanical devices) Transition Coord must be able to move frequently throughout the work day Must be able to speak and write the English language in an understandable manner Must be able to cope with physical and emotional stress of the position Must posses sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirement of this position can be fully met Must meet the general health requirements set forth by the policies of this facility which include a medical and physical examination Must have the ability to work effectively with residents and personnel. Must be able to push, pull, move, and/or lift a minimum of 25 lbs. to a minimum height of 3ft. and be able to push, pull, move, and/or carry such weight a minimum distance of 10 feet.
