An exciting opportunity for a social worker to join our Adult Social Care Mental Health Team, leading and supporting on our Transitions Pathway for young people.
You’ll primarily work with young people from the age of 16 who are neurodiverse and or are presenting with complex emotional and mental health issues. You will support them as they transition from children’s services into adult services, working alongside other agencies and partners to produce positive outcomes. Reporting to the Community Mental Health Team you’ll attend transitions meetings, undertake assessments and coordinate care and support. This role includes working with Children’s services, you will also need to establish relationships and draw from the expertise and knowledge of specialist services such as Mental Health, Neurodivergence, Specialist CAMHS, Substance Use, Education and Housing.
We welcome applications from newly qualified social workers. This role would suit a social worker with an interest in working with young people and have an interest in mental health and supporting transition pathways.
As a newly qualified Social Worker, you will have the best start with dedicated support to grow into your social work career. We also recognise the immense value of experience, and the complex work experienced Social Workers undertake. You will have the opportunity to progress in social work practice (outside of a management pathway) via our ‘Experienced Practitioner Progression Framework’ * Successful progression offers the higher range salary reward**

The ability to travel within the county is essential.

  • The Experienced Practitioner Progression Framework recognises Social Workers who have completed their consolidation programme, and who routinely undertake complex social work practice. It also recognises those Social Workers who undertake relevant post qualifying, CPD programmes and research, which could include being a Practice Educator who has 2nd and 3rd year Students, or an AMHP licenced and practicing within the Authority.

Progression beyond pay point £42,839, up to the maximum of £47,181 is subject to a progression panel assessment under the Authority’s ‘Social Work Experienced Practitioner Progression Framework
