A BIT ABOUT THE ROLE
Are you passionate about supporting young individuals in their journey towards independence? We’re looking for a dedicated Transition Worker to assist young people transitioning from care to independent living or back home. This role involves collaborating closely with youths, their families, and various agencies to ensure tailored support is provided. Responsibilities include nurturing essential life skills, facilitating access to education and well-being services, and guiding them through the complexities of adulthood.
Our Transition Worker role, although sitting within our family support service, is going to predominantly work with young people who have been in care transition out of care and into their own tenancy. There will also be an expectation and requirement to work with families - including on some parenting skills and emotional regulation for parents, to ensure stronger outcomes in terms of returning home, where appropriate.
You’ll work across a range of community setting’s, including our own residential services as well as Moray Councils.
The service delivery model offers preventative interventions to young people and families in their homes and communities, helping them identify needs and develop strategies to address them. The core theme of emotional, physical, and social wellbeing guides all service activities, with a focus on keeping children safe and placing families at the heart of decision-making processes.
Some key responsibilities of the Transition Worker role will be:
