We are seeking a full-time Transitional Healthcare Facilitator for needs at Correctional Industrial Facility in Pendleton, Indiana. Available Shift: full-time days, Monday-Friday Pay Rate $25 per hour + Benefits! The Transitional Healthcare Facilitator will be responsible for the development and implementation of transitional healthcare plans for incarcerated individuals scheduled for release. The Transitional Healthcare Facilitator will coordinate with the Indiana Department of Correction and other state and local agencies to ensure that pre-discharge planning and preparation is accomplished, and those re-entering society are provided with necessary services and resources. Qualifications • Bachelor’s degree in psychology, social work, counseling or related field from an accredited institution required • Two (2) years of experience working in a correctional setting • Experience with/knowledge of community and state programs and services • Two (2) years of experience in re-entry services or release planning preferred • Must show/obtain current Purified Protein Derivative (PPD) documentation for tuberculosis screening • Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR) and Automated External Defibrillator (AED) use • Must be able to pass background investigation and obtain agency security clearance where applicable We offer excellent compensation and comprehensive benefits for our full-time team members including: Health, dental, vision, disability and life insurance 401(k) with company match Generous paid time off Paid holidays Flexible Spending Account Continuing Education benefits Question?
