Position: Transitional Housing Coordinator, Temporary
Duration: Minimum 6 months
Work Schedule: Monday-Friday 8am - 4pm

CASE MANAGEMENT ABILITIES

· Compiles, maintains, and ensures completion of all required documentation in electronic and physical case files, in accordance with the required standards and guidelines
· Provides and maintains the appropriate databases for tracking progress and statistics of each client for monthly reports to track milestones of transitional housing interventions, services and supports as clients progress towards independent living
· Provides transportation to attend programming events within the community
· Facilitates life skills and career-focused group workshops through face to face or virtual platforms through service delivery/and online discussions, workshops, assessments, and surveys in accordance with established guidelines
· Determines eligibility for financial supports and services for clients wishing to access funded training through third-party contractors
· Facilitates client access to the drop-in computer lab to provide assistance in navigating pre-employment, employment or labour related websites progressing towards a labour market attachment and or funding for occupational or skills training
· Advocates for and refers clients to career and community services within the community, career opportunities and provides monthly house meetings, supporting personal and career-related interventions and services
· Organizes and regulates special events including workshops, career fairs and hosting guest speakers by performing duties such as scheduling session times, booking facilities, and acquiring materials
· Provides services that target areas such as gaining work experience, training, skill enhancement, self-employment opportunities, and job search support
· Performs other related duties as required

SKILLS AND ABILITIES:

· Strong understanding of BC Housing transitional housing programming
· Understanding of the Transtheoretical Model of Change Readiness
· Working knowledge of current employment issues, life/employability skills, the current landscape of local labour markets, trades and apprenticeships, harm reduction and trauma informed practice
· Thorough understanding of Canadian standard guidelines for career development practitioners
· Extensive experience working with multi-barriered men and community partners, specifically providing services and supports within the human services and/or employment services sector
· Excellent communication skills, both verbally and written
· Proficient in MS Office, Office 365, FYIdb or similar client databases
· Experienced in adult learning theory, motivational interviewing
· Ability to facilitate from a First People perspective
· Ability to navigate various social media platforms, online employment websites and applications
· Ability to facilitate groups
· Ability to write funding/training applications
· Ability to organize and prioritize work
· Physical ability to perform the duties of the position

QUALIFICATIONS & REQUIREMENTS:

· Certificate or Diploma in employment and career development, or a related social services field
· Minimum 2 years’ experience working directly with individuals with employment barriers, substance use disorder and/or complex barriers within a transitional housing program
· Proven experience facilitating one-on-one workshops on life, pre-employment and employment skills depending on individual needs
· Certified Career Development Practitioner (CCDP) an asset
· Valid Class 5 Drivers’ License and clear driver’s abstract
· Occupational First Aid Level 1
· Criminal records check and CSC Reliability clearance

Responsibilities
  • Under the supervision of the Senior Manager, Strategic Operations and Community Services, interviews clients, gathers background information, determines client suitability for the transitional housing program, informs clients of transitional housing program policies and procedures
  • Facilitates in-house group sessions in accordance with established client transitional housing programming
    · Advocates for clients by problem solving in areas such as accessing community resources

· Provides crisis intervention in situations such as medical emergencies and client disputes

  • Coordinates the maintenance, cleanliness, and safety of the client in the transitional housing program
  • Ensures the client’s inventory of move in supplies are maintained
  • Collects monthly rent and ensures clients are current and up to date on their rent
  • Works closely with Ministry of Social Development and Poverty Reduction to ensure clients are registered with the Ministry (as required)
  • Assists client’s in registering with the Food Bank
  • Ensuring clients are accessing onsite medical, mental health services and pharmacy services
  • Reviews and maintains related documentation as per BC Housing standards and guidelines
  • Acts as a liaison with community agencies, and promotes and encourages community involvement
  • Contributes to team practices and participates in planning, program development and policy recommendations
  • Facilitates rent reviews
  • Ensures information is uploaded into the BC Housing portal
