WHAT YOU WILL DO:

  • Build client capacity, empowerment and create self-reliance
  • Work collaboratively with a multidisciplinary team to move clients though a continuum of services to decrease wait times
  • Develop and initiate relationships with prospective clients based on trust, respect, and honesty
  • Work in collaboration with the Peel Outreach Support Landlord Engagement Specialist to support successful transitions and assist with housing stability
  • Support the transition of persons into housing utilizing a Critical Time Intervention Approach to Housing-Based Street Outreach. Set short and long-term goals with persons involved and create and document support plans (including Coordinated Care Plans) to reach those goals. Make additional referrals as appropriate
  • Complete initial intake assessments and discharge assessments at the appropriate date(s) and other documentation as required
  • Connect individuals with housing, including accompanying them to view potential housing options and being present on move-in day
  • Conduct Case Conferencing with support system of persons involved in the program
  • Maintain professional relationships with referral sources and other service providers
