Transitions of Care Call Center RN/LPN General Summary/Responsibilities: The Transitions of Care Call Nurse is a Registered RN/LPN nurse and is responsible for the provision of care through the Transitions of Care telephone and Remote Patient Monitoring systems as appropriate. In providing timely and appropriate clinical guidance, the transitions of care clinician initiates and transitions nursing care by assessing and evaluating the patient/caregiver understanding and adherence to the discharge instructions received upon discharge from a PHP or other medical facility. This may include acute care hospital and urgent care facilities. This position requires flexibility, strong clinical knowledge base, critical thinking skills, independent decision making and collaboration with the patient’s health care team. Additionally, the Transitions of Care Call Nurse assists in facilitating accessibility to identified patient needs, including PCP hospital follow up appointment, home care services and ongoing care management. Requires flexibility, accountability and may require ability to work independently. Requires strong computer skills, knowledge of Microsoft Office, Horizon, Remote patient monitoring systems, and the EPIC electronic health record system. Qualifications Education Minimum Level of Education Required: Associate degree Additional requirements:  Type of degree: Associates  Area of study or major: Nursing Licensure/Certification/Registration  Current Ohio nursing licensure (RN or LPN) in good standing.
