The Renal Transplant Team is seeking an enthusiastic individual for the post of Transplant Nurse Practitioner. The successful candidate will be working as part of the renal team within the South West Transplant Centre. We are a regional transplant unit responsible for coordinating the care of living kidney donors, and potential renal transplant recipients. We also provide long term follow up care to patients who have received a kidney and/or pancreas transplant. This is a rotational post within the roles of working in the post-transplant role, the pre-transplant role and a supporting living donor coordinator role. Rotations can occur every 2-3 years or sooner depending on service management.
The successful candidate should have demonstrable experience working within the area of transplantation and/or in a health promotion/screening role or in another specialist area managing their own caseload as renal teaching will be provided to undertake this role.
Working hours 37.5 hours following across Monday–Friday 8am-4pm.
Participate in a 1:6 on call for national kidney offers.
Preference will be given to internal Trust staff, as well as ‘Priority’ and ‘At Risk’ status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.
Assist in the provision of a regional specialist transplant programme. The geographical area covers the Southwest Peninsula with a population of 2.2 million covering the areas of Cornwall, Plymouth, and the South of Exeter.
Assess, plan, implement and evaluate expert nursing care and provide Specialist nursing support to potential live donors and recipients, as well as their carers within the Southwest Peninsula. Under the supervision of the live donor co-ordinators the post holder will have continuing responsibility for the co-ordination and organisation of individual treatment programmes throughout the living donor pathway and work to develop communication across divisional, regional, national, occasional international and organisational boundaries.
Manage the care of potential transplant recipients being assessed for transplantation by maintaining the transplant waiting list. Manage new and ongoing referrals, facilitate the multidisciplinary team meetings, and organise the transplant assessment clinic. Provide annual consenting for recipients active on the waiting list.
Provide follow up care and education to recipients who have received kidney transplants, pancreas transplants and combined kidney and pancreas transplants. Facilitate the transplant follow up clinics.

Manage and coordinate new and existing referrals for patients who are suitable for transplantation, enabling discussion in the multidisciplinary team meeting and liaising with the referral centres.
Maintain the transplant waiting list by regularly reviewing the active, suspended and pending lists facilitating regular meetings with the referring consultants to ensure suitability and enhance timely activation or reactivation.
Prepare and coordinate transplant assessment clinics locally and regionally, providing an education session for potential recipients and their families.
To ensure patients who are on the transplant waiting list are annually consented, keeping their contact details up to date, confirming the process of accepting an offer and revisiting the option of live donation.
Commence education programme for new transplant recipients following agreed educational packages and guidelines. Provide education for renal transplant patients and their families/carers pre and post transplantation. Provide psychological support pre and post transplantation . Effectively triage both routine and acute referrals being a first point of contact for transplant patients. Triage telephone enquiries and manage the day to day running of the transplant office. Take part in the in- reach service.
Provide nurse led clinics for new and established patients in both routine and emergency situations.
Coordinate and facilitate anaemia management of transplant patients including intravenous iron administration and monitoring and review of erythropoietin.
Provide annual reviews for post-transplant patients based on national polices and standards. Keep up to date in changing guidelines and evaluate the service provided. Provide screening and health education, both within Derriford hospital and travelling to other areas to provide an outreach service.
Provide advice and support to patients who have a failing transplant, facilitating timely referral back to the chronic kidney disease team for appropriate education.
Participate in a 1:6 on-call rota to cover donor referrals and kidney offers from NHSBT, 24 hours a day, 365 days a year, coordinate the process from initial referral to the admission of the patient. Liaise and refer to all relevant departments and members of the multi-disciplinary team to deliver a planned and seamless service.
To undertake venepuncture following hospital trust guidelines.
To communicate effectively with other organisations and healthcare professionals including but not limited to general practitioners, referring centres and regional transplant units.
Coordinate patient investigations and results for review by medical practitioners.
Complete UKT documentation for the pre, post-transplant patients and live donors. To maintain clear and accurate records using the vitaldata database ensuring the appropriate use of all clinical systems.
Carry out basic statistical analysis on data collected.
Present data at internal and external meetings.
To complete the transplant nurse practitioner/ live donor competencies.
To complete all NHSBT documentation.
To communicate effectively with other organisations and healthcare professionals including but not limited to general practitioners, referring centres and regional transplant units.
To maintain their own professional development, attending courses/further training to enhance and optimise his/her role within the work area.
To participate in the training and development of junior staff on the ward and participate in the supervision and support of student nurses and new members of staff joining the renal unit.
To promote and safeguard the wellbeing of patients, their families, and colleagues.
To participate in the development and implementation of ward area policy and nursing strategies.
