In this dynamic role, you'll deliver direct patient care and provide support to families, while also stepping into temporary assignments at various branches as needed due to staffing gaps or census increases. This is a unique opportunity for an RN looking for flexibility, variety, and purpose-driven work across multiple locations. Key Responsibilities: Provide direct nursing care and manage hospice patients’ plans of care Support and lead interdisciplinary care teams including LPNs, CNAs, and support staff Assess and address patients’ physical, emotional, social, and environmental needs Document care thoroughly and accurately Supervise and guide HHA/LPN staff Educate patients and families about medications, care needs, and safety Participate in on-call rotation and provide after-hours support as assigned Collaborate with physicians and attend interdisciplinary team meetings Assist in referrals, resource connections, and compliance with payer requirements About You What You’ll Need: Active RN license in the state of employment Minimum of 1 year RN experience (acute care, oncology, long-term care, hospice, or home health preferred) CPR certification Valid driver’s license and current auto insurance Reliable transportation Preferred Skills and Qualities: Strong hospice and end-of-life care knowledge Excellent clinical and assessment skills Compassionate, supportive, and calm in crisis situations Effective communicator with patients, families, and interdisciplinary teams Strong organizational and documentation skills Flexibility and resilience in a dynamic work environment We Offer Schedule & Travel: This role typically follows a Monday–Friday schedule, with occasional on-call responsibilities. Travel is required and may include: Option 1: "Overnight travel required to cities within a 100-mile radius." Option 2: "Frequent overnight travel to multiple states is required."
