Job responsibilities Please refer to the Job Description and Person Specification attached as PDFs to this advert. Main Duties and Responsibilities of the Post To plan and deliver stop smoking advice and support sessions, including advice on stop smoking medications, working within recommended protocols and guidelines. This may involve individual, telephonic, on-line and group work sessions in a variety of settings and venues. 2.
Identify and approach service users who are eligible for tobacco dependence conversations and treatment in a variety of different settings. These may be in a clinical area, virtually or in a persons home. Follow service users through their engagement with the service to the location of their care provision in order for integrated care. 3.
Develop individually tailored support plans including recommending treatments based on assessed needs. 4. Provide general health, lifestyle advice and support to people using the service e.g. healthy eating, physical activity, stress management.
This may include individual and group-based interventions, signposting and referring to other health, community, and voluntary services. 5. Participate in educating pregnant service users, their significant others and families about the benefits of stopping smoking in pregnancy and give support to those service users, significant others and families wanting to quit. Promote smoke free homes and support and advise service users, significant others and families about passive smoking.
6. Deliver Carbon Monoxide (CO) monitoring to a high standard and engage in the data collection and collation of CO rates in pregnancy and post-partum. 7. Engage with the targeted Smoking Cessation intervention, keeping accurate records and enable good data collection.
Maintain accurate up to date and comprehensive records at all times. 8. Ensure that the relevant equipment is provided and maintained on an agreed schedule. Report any failings of equipment or other environmental factors to the Clinical Lead.
9. Respond effectively to referrals within stated timescales self-referrals from the individuals, and those from colleagues/other services. 10. To be involved in the monitoring and evaluation of patient/client feedback, offering recommendations as part of the team to improve services.
11. Be aware of how to report incidents, concerns or complaints and escalate any concerns or complexities to the Clinical Lead Monitoring and Administrative Tasks: 1. To contribute to the monitoring and evaluation of the service, including the completion of the minimum data set requirement. This will include following up quitters who do not attend for planned support.
2. Maintain accurate patient and staff records in line with Trust guidelines. 3. Ensure administrative duties are carried out efficiently and effectively in order to meet the needs of the service
