Description POSITION SUMMARY The Treatment Coordinator is responsible for the coordination of clinical and person-centered planning services within Kelberman’s Clinical Department. The Treatment Coordinator manages the provision of all treatments, activities, and experiences related to Kelberman’s Neurodevelopmental Diagnostic Evaluations, Psychotherapy services, and Therapies (e.g., speech, OT, PT), as prescribed through the admission process and each individual’s treatment plan. The Treatment Coordinator participates in treatment/program planning and collaborative partnerships and initiatives for the purpose of strengthening the infrastructure of supports available for inclusiveness and quality. The Treatment Coordinator also functions as a liaison between Kelberman and other providers of services/care coordinators for each individual supported by Kelberman’s Clinical Department. Requirements ESSENTIAL RESPONSIBILITIES Activities include but are not limited to the following: · Serve as a liaison and primary contact for families, including access to all appropriate services within the Article 16 clinic and community providers by providing information and coordinating transitional guidance and support, as appropriate. · Support individuals and families throughout the admissions and treatment process. · Maintain a record of participants, person centered planning outcomes, and recommendations. · Communicate with families at the start of the process for diagnostic, psychotherapeutic, and therapy-based services. · Assist clinical team with determining appropriateness of referrals and refer to Kelberman’s Family Navigation when necessary. · Document all communication with family/individual in appropriate location. · Complete admission plans and input diagnostic info in the EHR. · Monitor/track treatment plans and ensures completion of timely treatment plans by the clinician · Monitor/track timely completion of diagnostic evaluation reports by providers. · Review intake and admission procedures and obtain any necessary and past information from referral sources. · Communicate required information to referral sources, the service coordinator and/or other interested appropriate parties. · Assist with insurance-based needs-including submission of preauthorization’s, follow-up on preauthorization’s, and inquiring into eligibility for unique insurance situations. · Ensure clinicians have completed their portion of the preauthorization in a timely manner. · Track and facilitate resolution of prior authorization denials from insurance. · Maintain clinical records, obtain all necessary consents to ensure regulatory compliance. · Prepare, complete and submit treatment plans, addendums, and discharge plans. · Communicate regularly with supervisor related to managing waitlist, referrals and etc. EDUCATIONAL REQUIREMENTS/ QUALIFICATIONS/ EXPERIENCE · Bachelor’s degree in Human Services or related field or license as a registered nurse · At least 1-year experience working with people with developmental disabilities and/or behavior management is required. · Basic Computer skills (Windows, Outlook, Adobe Acrobat, Word, Excel, Access). An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
