Description JOB TITLE: Triage & Chronic Care Nurse Manager DEPARTMENT: Triage REPORTS TO: Clinical Administrator OVERALL SUMMARY: The Triage & Chronic Care Management (CCM) Nurse Manager is a registered nurse responsible for overseeing the CCM nursing and clinical support staff. The Triage and CCM Manager plays a crucial role in overseeing the coordination of complex patient care, ensuring seamless transitions between healthcare settings, and advocating for the needs of our patients. The Triage & CCM Manager is directly accountable to the Clinical Administrator and works collaboratively in responsibility. DUTIES AND RESPONSIBILITIES: 1. Supports the mission and philosophy of Rockford Gastroenterology Associates. 2. Supervises staff in day-to-day functions, including training and development of the team to accomplish program goals and objectives. 3. Routinely takes an assignment and works along side of the team, especially during periods of staffing shortages. 4. Works as part of the healthcare team to assess patient needs, plan and modify care and implement interventions. 5. Works collaboratively with Clinical Administrator in areas of hiring, terminating and employee evaluations. 6. Responsible for monitoring the orientation of all new personnel in the department. 7. Schedules employee in-services and conducts skill verification with Clinical Administrator 8. Attends and is an active member of Joint GI Lab Meetings and Quality Committee 9. Prepares staff work schedule. 10. Is a member of the Clinical Policy and Procedures Committee and actively participates in clinical policy review and updating. 11. Meets with sales representatives as it pertains to nursing team. 12. Directly evaluates the quality of care given to patients in Triage and CCM through personal observation, patient interviews, and the use of other evaluative tools. 13. Conducts yearly performance evaluations and goal review with each staff member. 14. Initiates coaching and/or corrective action when necessary, per the organizations policy and keeps the Clinical Administrator and Human Resources Director updated. 15. Directs staff members in developing an effective, therapeutic plan of care through the use of the nursing process (planning, implementing, evaluating, and modifying the plan as directed). 16. Directs and evaluates, with staff members, the patient’s ongoing educational process. 17. Confers with providers to resolve problems. 18. Works collaboratively with other departments. 19. Responds appropriately to drills, reviewing procedure periodically with staff to ensure knowledge. BEHAVIORAL EXPECTATIONS: 1. Requirements Qualifications: Current Illinois State Registered Nurse licensure Graduate of an accredited school of nursing program Current BLS and ACLS Certification Experience in Gastroenterology preferred Experience in directing, teaching and supervising personnel
