Job Details Level: Experienced Job Location: Pasadena - Pasadena, MD Position Type: Full Time Education Level: 2 Year Degree Salary Range: $35.00 - $46.00 Hourly Travel Percentage: None Job Shift: Day Job Category: Health Care Description The Triage Nurse is responsible for supporting and maintaining communication between patients, families, case management nurses, field teams, and the Rapid Response team. Triage coordinates Skilled Nursing Visits (SNVs) for those patients and families that need a visit or request a visit. Office Location: Pasadena or Waldorf, MD - This position is a hybrid position with some in office time required. Status: Part-time Schedule: 2 12-hour shifts, Rotating Days, 8:00 a.m. to 8:30 p.m., with rotating weekends. Salary: Competitive, commensurate with experience, education and credentialing. DUTIES AND RESPONSIBILITIES Triages calls and routes the caller to the appropriate resource. Receives verbal orders from Physicians (i.e. medications, DNR status and EMS forms) and updates the plan of care. Places orders for medication and DME (durable medical equipment) based on patient needs and physician orders. Instructs families on how to manage patient’s symptoms using the HSK or other medications listed on the patient’s medication list until the PN can be contacted or a Skilled Nursing Visit (SNV) can be arranged if necessary. Provides support to the Clinical Staff working in the field by being a resource for palliative care options, symptom management alternatives, clarification of the formulary and our symptom management protocols. May be required to go out into field to conduct Skilled Nursing Visits, as needed. Field visits may include visits to patients, in facilities, hospitals and home settings, including visits to our pediatric population. Qualifications Your Qualifications: Possess a current Maryland nursing (RN) license and thorough knowledge of nursing principles with a minimum of 2 years of experience. Must have excellent communication skills and be able to assess patients’ needs (i.e. physical, teaching, mental and spiritual) and give direction after speaking with the caregiver on the phone. Ability to recognize safety issues to self and others and be able to assess and implement use of lifts or other personnel to lift and move patients of all sizes and weights in a safe manner. Ability to perform consistently in stressful situations. Strong and creative problem-solving skills with the ability to prioritize and delegate. Basic computer skills, to include but not limited to Microsoft Word and Outlook, required. Prior experience working with EMR (electronic medical record) system preferred. Bilingual in Spanish and English highly preferred. Demonstrated commitment to quality, timely, and cost-effective patient care. Ability to simultaneously handle multiple assignments and projects with attention to details, efficiency and accuracy and deadlines. Responsibilities

The Triage Nurse coordinates communication between patients, families, and healthcare teams, ensuring timely Skilled Nursing Visits. They triage calls, manage medication orders, and provide support for symptom management and palliative care options.
