Health Department· Membertou, Nova Scotia
Title: Unama’ki Diabetes Lead
Description: Reporting to the Director of Health, the Unama’ki Diabetes Lead supports Unama’ki Mi’kmaw communities in a public health capacity, supporting diabetes prevention and management initiatives. This role will explore strategic action in development of diabetes prevention and management programing.
Location: The Diabetes lead will be required to travel regularly throughout Unama’ki/Cape Breton and occasionally within mainland Nova Scotia and nationally.

REQUIREMENTS:

  • Active license with the Nova Scotia College of Dietitians and Nutritionists or with the Nova Scotia College of Nursing
  • Certified Diabetes Educator (CDE) certification or willingness to obtain
  • Proficient with online communication and file management
  • Knowledge of Indigenous and/or Mi’kmaw communities/organizations or willingness to learn
  • A valid driver’s license and access to a reliable, insured vehicle is required.

PREFERRED EXPERIENCE:

  • Previous experience in a health care setting
  • Experience working with Mi’kmaw or other Indigenous communities considered an asset
  • Understanding of Mi’kmaq or Indigenous ways of knowing considered an asset
  • Knowledge of diabetes care and prevention
  • Strong program development and evaluation skills
  • Ability to speak and/or understand the Mi’kmaw language is an asset
  • Strong understanding of the social determinants of health

Responsibilities
  • Work in collaboration with Unama’ki Mi’kmaw community health centers, provincial health authorities, diabetes educators, health navigators, and healthcare organizations
  • Work with a team to develop a plan for mapping existing diabetes management and prevention services, initiatives, or policies serving the Unama’ki/Cape Breton Mi’kmaw communities
  • Identify culturally appropriate approaches to diabetes prevention and management that have been successful for Mi’kmaw communities in Unama’ki
  • Work with a team to develop a plan for engaging communities to identify priority areas for supporting prevention of diabetes and diabetes related health outcomes
  • Help create culturally appropriate tools and resources to support community members, health care workers/navigators, and health centers with navigation of diabetes services.
  • Develop culturally appropriate tools and resources that can support communities to evaluate the success of their diabetes programs and services
  • Work with a team to support Mi’kmaw youth living with diabetes through collaborative programing such as land-based camps
  • Attend community events to offer resources related to diabetes
  • Stay current with Diabetes Canada guidelines and best practices
  • Promote culturally safe care and interprofessional collaboration
  • Write and assist with funding proposals, workplans, and budgets
  • Complete required project/funding reports
