Wage Schedule: $43.190 - $60.120 per hour

Reporting to the Patient Care Manager in Emergency, The ED/Med Admits Unit coordinator is responsible for engaging in daily discharge rounds for admitted patients within the Emergency Department- maintaining thorough output, identifying and facilitating appropriate discharge planning and identifying and facilitating the transfer of patients to other sites as appropriate and required. The incumbent will work closely with the inter-disciplinary team including TM, GEM, SW and the UC in the ED maintaining data accountability and acting as a steward for developing and facilitating the family-centered care approach

QUALIFICATIONS:

  • Registration with the College of Nurses of Ontario (in good standing);
  • Current BCLS; ACLS with Medical Directives required;
  • Recent & related Charge Nurse Experience required (must be within the last 2-3 years);
  • Significant and recent (within one year) experience in an Acute Medical Unit, required;
  • Recent experience in Emergency/Critical Care/Intensive Care nursing preferred;
  • Proven experience with staff scheduling (ONA and CUPE), patient flow and patient assignments;
  • B.Sc.N, preferred;
  • Demonstrated leadership, teaching and mentorship skills;
  • Effective organizational; planning; prioritization and time management skills;
  • Demonstrated ability to deal with multiple competing demands with ability to maintain composure in stressful situations;
  • Proven ability to work well independently and to demonstrate well developed critical thinking skills;
  • Ability to collaborate with Lakeridge Health Patient Flow, the inpatient clinical program interdisciplinary teams and with community partners for quality based patient outcomes (ie. COPD Care Pathways)”.
