MAIN DUTIES OF THE JOB
Provide specialist nursing advice to patients referred for specialist intervention care and support within the services, being a point of contact for assessment, information and advice for the patient, family and carers. In liaison with others support the development of staff caring for patients through support, formal education, practice development and role modeling. Manages own nurse-led outpatient clinics, including vaginal pessary management, bladder retraining, patient education, catheter care, and medication review. In conjunction with other relevant clinical staff, you will assist with clinical development in maintaining and improving all aspects of clinical governance. Use a range of techniques to ensure and monitor standards of clinical practice, audit, research and teaching to continually improve standards of care and patient experience. Will ensure cost effective use of all resources within appropriate governance frameworks. Attend and contribute to MDT meetings. Follow-up of patients as appropriate and ensure relevant information is provided within your own sphere of competency and refer to other members of the team as appropriate.
DETAILED JOB DESCRIPTION AND MAIN RESPONSIBILITIES
To assist and support the Urogynaecology Team in the delivery of specialist services for women with pelvic floor dysfunction across the acute and community settings as required. Provide an effective and patient focused Urogynaecology service which is considerate and compassionate. Organise, prioritise and evaluates workload based on clinical skills and competencies in line with operational and clinical protocols. Provide evidenced based clinical, social and psychological support to their patients using excellent communication skills, education and health promotion to enable them to reach their optimum level of independence and health. To provide information, advice and support to their defined patient caseload, nursing peers and junior staff. Work with others to develop an integrated service liaising with other departments and professionals in the development of care systems, protocols and guidelines. Work as part of a multi-disciplinary team developing excellent interdepartmental relationships with excellent communication with a focus on quality improvement. Provide information and support to patients, their families and other healthcare professionals following diagnosis and through treatment. Liaise with the Gynaecology in-patients and ward staff to enhance the patient journey when appropriate. Deal effectively and professionally with patient/service user issues collaborating with PALS as appropriate and take a proactive approach to resolving issues including learning from complaints and escalate when unable to resolve. Maintain accurate appropriate and timely patient records within agreed databases and electronic systems. Actively contribute to the multi-professional approach in the management of patients /carers within the Urogynaecology Unit. Ensure you work within an agreed divisional work plan and with awareness of own knowledge limitations and seek further advice or supervision as required. Actively contribute to any relevant initiatives within the Urogynaecology unit and provide support to the nurse consultant and senior clinical nurse specialist. Take responsibility for the health, safety and welfare of self in the working environment following safe working practices and to comply at all times with the Trust’s health and Safety Policies and Procedures. Comply with measures to actively manage and reduce risk to patients and ensuring the formal reporting and recording of adverse incidents in line with Trust policy
