JOB SUMMARY:

Reporting to the Manager, Patient Navigation & Utilization, the Utilization Resource Nurse who is a fully integrated member of a multidisciplinary team, to include Physician Advisor and Social Workers, is responsible to improve patient flow processes by optimizing best practice utilization strategies. The Utilization Resource Nurse is responsible and accountable for patient placement and bed utilization and ensures quality patient care and safety by providing bed access, appropriate patient placement and bed utilization within available capacity.
The Utilization Resource Nurse is responsible to support, promote, and lead through example, the adoption of the organization’s mission, vision and values. The Utilization Resource Nurse with the Occupational Health and Safety standards for the hospital and is proactive in promoting a safe work environment.

QUALIFICATIONS:

  • Current knowledge: Clinical utilization management, community resources and ability to access patient-client services
  • An advanced level of professional leadership with various members of the multidisciplinary team
  • Ability to establish and maintain positive working relationship within and interdisciplinary team using superior interpersonal skills
  • Excellent problem solving skills and the ability to think innovatively and independently and within a pressurized environment
  • Commitment to professional development
  • Ability to work in a collaborative team approach and to adhere to the accepted standards of personal and professional conduct.
  • Excellent written and oral communication skills.
  • Basic knowledge of Microsoft Office computer programs including Excel, Care spread sheets, Power Point, Word, Outlook.
  • Ability to flex work hours (weekdays and weekends)

EDUCATION/TRAINING EXPERIENCE:

  • Registered Nurse with current Certificate of Registration with the College of Nurses of Ontario – required
  • Minimum 5 years current experience in Acute Medical/Surgical Nursing – required
  • Previous experience in utilization management and discharge planning – preferred
