HOW WILL YOU CONTRIBUTE AND GROW?
Reporting directly to the Digital Value-Based Coordinator, the Person will be responsible for contributing to the development of valuable services offered to patients undergoing treatment. This involves defining and delivering personalized support plans and their corresponding clinical follow-ups, with the goal of improving the therapy experience and ensuring optimal therapeutic adherence.
The Value-Based Case Manager will manage and monitor data from the remote monitoring platforms of devices used for respiratory therapies (C-PAP, pulmonary ventilators, etc.), aiming to improve therapeutic adherence in less compliant patients through personalized follow-up plans and educational phone support.
