Description The Veteran Liaison is responsible for making professional, comprehensive presentations of the Company’s services to the NW Florida veteran community and in specific assigned territories to achieve awareness and recognition goals and increase veteran participation in the hospice benefit. This person is also responsible for all veteran education and coordination of the Transitional Care Nurses and Account Executives for the hospice division, will initiate a comprehensive plan of veteran recognition for all new veterans accepting the hospice benefit, and help hardwire veteran identification upon receipt of notification from intake department. The Vet Liaison is a resource and educator for facility staff, clients, and caregivers regarding veteran related services. Must have a broad military background and have been honorably discharged from the armed forces. This person will help facilitate bi-annual fundraisers for the veteran funds in Peoples Foundation of Northwest Florida, Inc. Requirements Essential Duties & Responsibilities Demonstrates consistent attendance and meets goals with minimal supervision. Is always prepared for scheduled appointments and meetings. Fosters a positive attitude, builds morale among staff, and promotes open communication with all departments. Participates in Company functions to build community and public awareness of Peoples Hospice and Palliative Care. Maintains working knowledge of Medicare, Medicaid, and private insurance (and managed care when applicable), as well as agency policies and procedures. Coordinates veteran service identification and utilizes a team approach as needed to recognize the veteran’s service. Makes pre-discharge evaluation visit to the patient, confers with other professional staff, and participates in instruction and supervision of patient and/or family members. Completes documentation accurately, timely, thoroughly, and in compliance with written procedures. Monitors trends that indicate need for new products or services and oversees product development. Determines the potential of community relationships to help gain access to military personnel to further educate on company veteran programs. Identifies the potential of each account and allocates time and effort according to military potential. Works with others to identify costs for alternative contract proposals while protecting margins. Identifies the customer’s primary needs and key concerns, and identifies key contacts and their roles in the decision-making process. Follows up on all proposals to ensure implementation. Maintains accurate records, documents actions, maintains control of expenses and submits required reports on time. Continues professional growth through attendance at workshops, professional in-services, etc. Maintains a professional image. Uses appropriate phone etiquette. Promotes a customer friendly atmosphere for all visitors and always ensures patient confidentiality. Participates in accreditation program. Performs other duties as assigned. Desires Qualifications: · Military veteran who has served his/her country with honor and valor. · Dependable transportation with valid driver’s license and auto insurance. · Proficiency using Microsoft Word, Excel, and PowerPoint. · Understanding of financial management. · Theoretical knowledge of Hospice and Palliative Care. · Able to prioritize and manage time effectively and make independent decisions when necessary. · Proven interpersonal, coordination, and leadership skills with ability to communicate effectively. · Proven leadership and team building skills · Ability to converse with Peers and Senior Managers · Ability to prioritize and manage time effectively and make independent decisions when necessary. Work Environment While performing the duties of this position, the employee travels by automobile and is exposed to changing weather conditions. The employee works in varied indoor and outdoor environments based on appointments and events.
