JOB DESCRIPTION:

  • Develop and execute a comprehensive vision and strategic plan for integrated whole-person care aligned to the mission of CCBHC. Stay abreast of industry trends, regulatory changes, federal/local practice requirements, and health management practices to ensure the CCBHC remains at the forefront of integrated care delivery.
  • Lead the design, implementation, operational management and quality oversight of new and existing integrated care programs and activities for identified medical risk populations. This includes integrating behavioral health, addiction recovery, primary care, and social support services into purposeful outcome driven clinical grounded protocols.
  • Oversee the day-to-day operations of integrated care services at the 5 CCBHCs, ensuring efficient workflows, appropriate staffing levels, and adherence to quality standards. Monitor key performance indicators (KPIs) to track program effectiveness and make data-driven decisions.
  • Recruit, train, and manage a high-performing team of clinical staff (RNs/APRNS/NPs) dedicated to integrated health management goals and provide health guidance across departmental partners regarding health risks and health management activities/goals. Foster a culture of collaboration, innovation, and continuous learning across departments.
  • Build and maintain strong relationships with internal departments, external healthcare providers, community organizations, and government agencies.
  • Ensure all programs and services comply with federal, state, and local regulations, including CCBHC certification requirements. Contribute to quality improvement initiatives to enhance patient safety, clinical outcomes, and overall service delivery.
  • Utilize data to inform health management strategy appropriate to the needs and risk of the population served.
  • Participate in the development and completion of grant proposals for submission.
  • Collaborate with an interdisciplinary team which includes therapists, care coordinators, clinical pathway coordinators, certified peer specialists, mobile crisis clinicians, psychiatric nurse practitioners and psychiatrists to assist with an integrated person-centered goal planning and a whole person approach.
  • Follow policies, procedures, and protocols to ensure compliance with federal and local requirements as a CCBHC and Integrated Outpatient Service (IOS).
  • Complies with billing and reimbursement requirements, maximizing revenue potential while maintaining program integrity and monitoring opportunities to advance clinical activities.
  • Comply with all relevant regulations, accreditation standards, and reporting requirements.
  • Participate in audits/reviews to ensure adherence to program requirements and standards.
  • Complete agency training to improve clinical competency and trauma informedcare as they align to CCBHC and Agency specific mission.
  • Perform other work-related duties as assigned.
  • Available to travel/work evening and weekend hours as required by the agency.
  • Work collaboratively with the CCBHC Interdisciplinary Team as well as external agencies and community stakeholders.
  • Attend required management and staff meetings. Complete CCBHC and organizational training.

EXPERIENCE:

  • A minimum of 5-10 years of progressive leadership experience in a healthcare setting, with at least 3-5 years in a senior operational management role overseeing integrated or whole-person care programs.
  • Experience working within a CCBHC is a significant plus.
  • At least 2 years’ experience with quality management and utilizing data to inform treatment for individuals with multiple chronic diseases.
  • At least 5 years working with an interdisciplinary team in a health focused role.
  • Must have at least 3 years’ experience as a direct supervisor.
  • Must have at least 3 years’ experience with operationalizing health management protocols.
  • Knowledgeable about the psychosocial aspects of illness, health and recovery.
  • Ability to work efficiently in a fast-paced environment.
  • Proficient with word processing, electronic health records, and electronic communications.
  • Well organized with strong time management skills.
  • Able to work independently and as a member of a team
  • Excellent interpersonal skills, including being able to maintain a professional temperament with both clients and colleagues.
  • Able to meet program standards in documenting in client records.
  • Ability to work with challenging clients.
  • Comfortable with community outreach and home visits.
  • Comfortable and competency working with racially, culturally, religiously and ethnically, linguistically, differently abled diverse communities, including but not limited to BIPOC and LGBTQ+ communities.
  • Bilingual skill in a language other English (i.e. Spanish, Russian or Cantonese) is desired.

QUALIFICATIONS:

  • Preferred: MD, DO, PhD, or Nurse Practitioner (NP) but will accept a MSN/MPH. A clinical background (e.g., MD, DO, PhD, PsyD, LCSW, LMFT, APRN) is highly desirable.
