Description

Case Managers provide comprehensive patient care education and case management services. Their work requires a working knowledge of social work, case management and local social services systems/resources. Their typical responsibilities may include handling case assignments, drafting service plans and coordinating and providing care, reviewing case progress and determining case closure; advocating for clients and helping them achieve wellness and autonomy; recording case information, completing necessary forms, and producing statistical reports. They may work independently or as a member of a team. They work under limited supervision, resolving most standard to moderately complex issues independently and referring highly complex or unique issues to a high level. Case Managers typically report to a manager or director or in some areas may report to an administrator.

Responsibilities

Responsible for the collaborative process with the client/patient/individual, family and other providers (health care, social services, school, etc.) of meeting the individual’s health and daily living needs through advocacy, assessment, planning, implementation, coordination, monitoring and evaluation of options and services Identifies potential high risk/complex cases and develops a case management plan that minimizes these risks Identifies and assesses community based services, educational, financial, mental health, and health care systems as it relates to the individual case management/care management plan
assuring their appropriate utilization Works closely with senior staff/manager to address problems and issues as it relates to the
provision of services Monitors patients’ health status, compliance with medical appointments, and monitor treatment recommendations from the medical provider on an ongoing basis Addresses the emotional and mental health care needs of the client and family affected by their diagnosis and ongoing medical treatment
